The thought record is one of the most widely used tools in cognitive behavioural therapy (CBT). It helps to map out the connection between a situation, automatic thoughts, feelings, behaviour, and consequences. This illustrated version is free to download and print as often as you like, for use in session, or to give to clients so they can continue practising at home.
Two variants are available. The standard thought record maps out the current situation. The thought record with helpful thoughts also provides space to write a helpful thought alongside the automatic negative thought. Seeing the effects of both thoughts side by side creates insight into the difference an alternative thought can make.
Both variants are available in white and black, and also in English.
What's on the form? Situation, automatic negative thought, feeling (with illustrated basic emotions: scared, sad, angry, and happy) and the intensity of the emotion, behaviour, and consequence.
