Overview

Here you will find illustrated Thought Record forms that you can download for free. There are two versions. One form features a 'traditional' Thought Record, where you examine the current situation. The other form includes space for helpful thoughts alongside the automatic negative thought. By seeing the effects of both the automatic negative thoughts and the helpful thoughts side by side, it provides insight into the impact of different thoughts. Both versions are available in two colors.


On the Thought Record, there is space to note the following points:

  • Situation
  • Automatic negative thought
  • Emotion (with illustrated basic emotions: Afraid, Sad, Angry, Happy) and intensity of the emotion
  • Behavior
  • Consequence

This Thought Record is a free download.

After downloading, you can print them as often as you like. You can also give them to clients so they can continue practicing at home.

The only thing I prefer you not to do is to change the files, cut out parts, or, for example, remove my website address. Enjoy (working with) them!
